At Polat Health, we believe in building clarity, trust, and transparency in everything we do. These Terms & Conditions outline the expectations, responsibilities, and boundaries of working together, so we can co-create a transformative and respectful coaching experience.

By booking a service, joining a program, or accessing our content, you agree to the following terms.

1. Who We Are

Polat Health provides personalized, integrative health coaching, functional nutrition guidance, and digital wellness resources. Our services are designed to educate, empower, and support positive lifestyle change, not to diagnose, treat, or cure medical conditions.

2. Scope of Coaching Services

Our coaching may include:

Lifestyle and mindset support
Behavior change tools and accountability
Nutrition and wellness education
Digital programs, resources, or check-ins

We do not provide medical advice, prescribe diets or supplements, or replace the care of a licensed medical provider. Clients with chronic or acute conditions are encouraged to work with their primary care physician alongside coaching.

3. Client Commitments

To get the most out of your experience, we ask that you:

Show up honestly and consistently
Provide accurate information on forms and during sessions
Participate fully and communicate clearly
Understand that transformation requires your personal commitment
Honor session times and boundaries

Coaching is a collaboration — you bring the readiness; we bring the support.

4. Payment Terms

Prices are listed in USD
Payment is due upfront or as outlined in your plan
We accept Stripe, PayPal, and HSA/FSA on eligible services
A Letter of Medical Necessity (LMN) can be provided upon request
Prices are subject to change, but active clients remain at their locked-in rate

5. Cancellation & Refund Policy

You may reschedule or cancel up to 24 hours in advance
Late cancellations or no-shows will be charged the full session fee
Digital products, downloadable files, and online programs are final sale
Coaching programs are non-refundable once the first session has been delivered
Session extensions or pauses may be requested on a case-by-case basis

6. Intellectual Property

All coaching content, resources, session materials, and programs created by Polat Health are our intellectual property.

They are for personal use only and may not be reproduced, resold, shared, or distributed without written permission.

7. Confidentiality & Privacy

Your information, forms, and session content are kept fully confidential. Only your coach and authorized team (if applicable) have access.
We do not share or sell your data — ever.

8. Technology & Access

Sessions are conducted virtually (Zoom or phone). You are responsible for having a reliable connection.
Recordings are only permitted with mutual agreement.
You may receive links to shared files or forms via Google Drive, Practice Better, or secure email.

9. Results Disclaimer

Coaching outcomes vary depending on your unique situation, commitment, and readiness.
We do not guarantee specific results or outcomes. What you receive from this experience is shaped by the energy you bring to it.

10. Legal Jurisdiction

These Terms are governed by the laws of the State of Texas, USA.
Any disputes shall be resolved in a Texas court of competent jurisdiction.

11. Updates to These Terms

We may update these terms as needed. The most current version will always be available on this page. Your continued use of our services indicates acceptance of any changes.
